Analysis of influencing factors and nursing countermeasures of delayed nausea and vomiting in children with tumor chemotherapy

Objective To explore the occurrence and influencing factors of delayed nausea and vomiting in children with tumor chemotherapy,so as to provide bases for doctors and nurses to formulate home care plans. Methods A prospective cohort study was conducted,and a toll of consecutive children who received chemotherapy and their caregivers in a tertiary A Children’s Hospital in Zhejiang Province from August to October 2022 were enrolled as the subjects of investigation. Baxter Retching Faces Scale and United States Department of Health and Human Services Common Adverse Event evaluation criteria were used to evaluate the incidence of delayed nausea and vomiting. The influencing factors of delayed nausea and vomiting were collected by questionnaire and analyzed by binary logistic stepwise regression. Results A total of 378 children with tumor chemotherapy were included,of which 229(60.6%) developed delayed nausea and 144(38.1%) developed delayed vomiting. Delayed vomiting mainly occurred 1-2 days after chemotherapy(50.7%). Logistic regression analysis showed that body surface area,negative emotion,history of delayed nausea and vomiting,and smoking environment were the influencing factors of delayed nausea in children with tumor chemotherapy. Body surface area,history of delayed nausea and vomiting,complicated with respiratory or digestive system diseases,vomiting risk of chemotherapeutic drugs,smoking environment and tumor type were the influencing factors of delayed vomiting in children with tumor chemotherapy (P<0.05). Conclusion The incidence of nausea and vomiting associated with delayed chemotherapy was high in children with tumor,especially at 1~2 days after chemotherapy. Large body surface area,negative emotion and history of delayed nausea and vomiting are the high-risk factors of delayed nausea in children with tumor chemotherapy;large body surface area,history of delayed nausea and vomiting,complicated with respiratory or digestive system diseases,high vomiting risk of chemotherapeutic drugs and solid tumors are high risk factors for delayed vomiting in children undergoing tumor chemotherapy. It is suggested that medical staff should formulate preventive measures according to relevant risk factors and carry out home care guidance to reduce the incidence of delayed nausea and vomiting in children undergoing tumor chemotherapy.

This article summarizes a series of organization and contingency management strategies of convertible ICU in severe rescue of COVID-19. Based on the 4S management model,focusing on the 4 core issues of space,staff,system,and supplies,the convertible ICU was quickly established through rapid transformation and expansion,overall sharing of equipment,and visual management of material placement. Personnel management includes establishing 3 reservoirs and 4 echelons,carrying out severe illness training,and carrying out classification authorization and combined shift scheduling. At the system level,a multidisciplinary collaboration mechanism was established and the ventilation gateway was moved forward in the prone position to improve the success rate of critical care. At the supply level,the key configuration of the intelligent monitoring system enabled efficient care,and psychological counseling support was provided to maintain employee’s mental health. During the period of the convertible ICU of rapid establishment and operation,through the implementation of these strategies,the timely,orderly,safe and efficient treatment of critically ill patients was ensured. The experience and theoretical basis were provided for the establishment and transformation of convertible ICU for other institutions in emergency situations.

A specific scale of quality of life in neurogenic bladder patients:Chinese translation,cultural adaptation and test of validation and reliability of the SF-Qualiveen

Objective To translate the SF-Qualiveen into Chinese,and to evaluate its reliability and validity in neurogenic bladder patients,to develop a specific quality of life assessment scale for neurogenic bladder patients. Methods After obtaining the authorization of the author of SF-Qualiveen,the scale was translated,back-translated and cross-culturally adjusted according to the Brislin model to form the Chinese version. 96 patients with neurogenic bladder in 5 tertiary comprehensive hospitals in Sichuan Province were investigated by snowball convenience sampling to evaluate the reliability and validity of the scale. Results The Chinese version of SF-Qualiveen included 4 dimensions and a total of 8 items. The decision value(CR) of each item was all>3.000,and the correlation coefficient between each item and the total score of the scale was all>0.400. The Cronbach’s α coefficient of the scale was 0.926,and the Cronbach’s α coefficient of each dimension was 0.908~0.931. The retest correlation coefficient of the scale was 0.971,and the retest correlation coefficient of each dimension was 0.539~0.935. The content validity index of the items(I-CVI) was 0.857~1.000,and the content validity index of the scale(S-CVI) was 0.875. The criterion validity of the scale was 0.891. Conclusion The Chinese version of SF-Qualiveen scale has been proved with good reliability and validity,and it can be used as an evaluation tool for the quality of life of patients with neurogenic bladder in China.

Application of early lower limb rehabilitation activities in patients with lower extremity deep venous thrombosis after catheter-directed thrombolysis

Objective To explore the effect of early rehabilitation intervention in deep venous thrombosis of lower extremity patients after catheter-directed thrombolysis. Methods 40 deep venous thrombosis of lower extremity patients admitted to hospital from June 2017 to March 2018 were enrolled in the control group with routine nursing care. Another 40 patients admitted to hospital from April 2018 to December 2018 were selected as the experimental group and received early rehabilitation nursing intervention. Upper and lower legs circumference,Urokinase dosage,thrombolysis time,comfort scores,pain scores,patient’s satisfaction,incidence of blood seepage at puncture site and catheter displacement,quality of life scores and PTS scores of patients at 1 month and 3 months after discharge were observed and compared. Results Leg circumference was significantly smaller in the experimental group than that in the control group. The thrombolysis time and Urokinase dosage were significantly lower in the experimental group than those in the control group.The patient satisfaction score in the experimental group was higher than that in the control group with statistical differences(P<0.05). The scores of physical functioning,physical role,physical pain,general health status,emotional role and mental health at 1 month after discharge were higher in the experimental group than those in the control group. The PTS score in the experimental group was lower than that in the control group with statistical differences(P<0.05). The scores of physical role,physical pain and general health status at 3 months after discharge were higher in the experimental group than those in the control group. The PTS score in the experimental group was lower than that in the control group. The differences were statistically significant(P<0.05). In the control group,the scores of 8 dimensions in QOL at 3 months after discharge were higher than those at 1 month after discharge,and the PTS score was lower than it at 1 month after discharge,with statistical differences(P<0.05).In the experimental group,the scores of physical functioning,vitality,social functioning and emotional role at 3 months after discharge were higher than those in 1 month after discharge. The PTS score was lower than it at 1 month after discharge. The differences were statistically significant(P<0.05). Conclusion Early rehabilitation intervention of lower extremity patients after catheter-directed thrombolysis can improve the quality of life and comfort of patients with deep vein thrombosis.
